Pictures of damage and BOL (delivery receipt) that is noted damages and signed by you and the driver MUST BE emailed to [email protected] BEFORE submitting this form. Please include your order number (PO#), tracking number (Pro #), full name, daytime phone number, vehicle model and make in your email. BOL may be faxed to 800-500-7767.
